At Southwest Key, we care about your physical health, which is why we encourage participation in a biometric screening to provide you with a health baseline that evaluates risks for various health issues. In addition, if you complete a biometric screening, you are eligible to receive a $25 credit towards your semi-monthly medical premium.

A biometric screening measures glucose, blood pressure, height, weight, and body mass index. By knowing and understanding your numbers, you can take preventive measures to lead a healthy lifestyle.

ON-SITE BIOMETRIC SCREENINGS

On-site biometric screening events are held at designated programs. Look out for our biometric screening communications and in-person presentations that will inform you of available on-site biometric screenings.

OFF-SITE BIOMETRIC SCREENINGS OPTIONS

Off-site biometric screenings are available all year-round.

There are currently 3 options to complete a biometric screening immediately. You can complete a biometric screening through your primary healthcare physician* (PHP), visiting a CVS Minute Clinic or through LabCorp Patient Services.

1. YOUR PHYSICIAN (PHP)

Complete your biometric screening with your primary healthcare provider (PHP).

2. CVS MINUTE CLINIC (not available in South Texas)

Download the voucher from the WellConnect portal and take it to your nearest CVS Minute Clinic for a quick and easy biometric screening. Voucher cannot be used at CVS Pharmacies.

3. LabCorp Services

Download a voucher from the WellConnect portal and visit your nearest LabCorp Patient Service Center for a quick and easy biometric screening.

The Healthy Lifestyle Credit, also referred to as the HLC is a $25 credit towards your semi-monthly medical premium. Employees can earn the semi-monthly credit by completing a biometric screening. To continue receiving the HLC, employees must complete their biometric screening every plan year. New employees will receive an email from the Wellness team offering directions and information relevant to offsite biometric screenings.

3 EASY STEPS

To Qualify & Earn The Healthy Lifestyle Credit

  
      STEP 1
      You must be enrolled in one of the Southwest Key medical plans: Base or Premium*. Health benefits are effective on day 1 of the date of hire.
    
      STEP 2
      Register in our Labcorp Biometric Screening Portal. Once your medical plan starts, you will receive an email from HR Wellness offering registration instructions on completing a biometric screening.
    
      STEP 3
      Complete a Biometric Screening. The $25.00 semi-monthly credit will be applied to your paycheck once it’s confirmed in our reporting system that you’ve completed the Biometric Screening.
    
      *Please note that there could be a delay (1-2 pay cycles) pending receipt of the data report on the completion of your biometric screening.

New hires will receive an email from Wellness offering instructions and information on how to complete a biometric assessment once Benefits verifies enrollment in the health plan
Medical benefits are offered at the time of hire and during the open enrollment period for eligible employees
Current employees must complete a biometric assessment each plan year by August 31 to continue receiving the Healthy Lifestyle Credit (HLC) for the following plan year
Southwest Key does not collect any individual results of your biometric evaluation and is not obligated to comply with the benchmark results related to your biometric evaluation
Individual results will not affect your eligibility for benefits
