A Message From Wellness
While we adapt to new and safe measures to limit the spread of Covid-19, including how to approach exercise and physical activity, it’s important to maintain optimum physical health during this time. We understand the challenges this presents especially, with closures and the limited capacity of many fitness centers and health clubs across the country. It’s easy for us to become sedentary when we are inside our homes for most (if not all) of the day. We recommend employees make accommodations to exercise inside their home, take advantage of virtual fitness classes, and take advantage of our wellness challenges when provided.
Fitness Membership Incentive
If my fitness center or gym is closed due to the coronavirus and I’m being assessed membership fees, do I still qualify for the incentive?
Maintaining your membership fee with your fitness center or health club is ultimately your responsibility. While the Southwest Key Wellness Program offers the fitness membership incentive, we are no longer waiving the eight calendar days of workouts per month required to claim your fitness membership incentive.
If you cannot meet the required workouts per month or your fitness center or health club is closed we recommend that you make arrangements to cancel or put your fitness membership on hold until your fitness center/health club reopens and has proper safety standards in place. To claim the fitness membership incentive, you must complete eight calendar workouts per month and submit proof.
If my fitness center or gym froze my membership account, do I still qualify to receive an incentive?
No. The incentive is only for those who are paying for membership fees.
If my fitness center or gym offers virtual workouts, do those qualify toward my required workouts to claim the Fitness Membership Incentive?
Yes. If your specific fitness center or health club offers virtual workouts as an option, you can apply those virtual workouts toward your incentive. You must submit all required documentation plus verification that you attended a virtual workout. The virtual workout must be with the fitness center or health club that holds your membership.
What do I need to submit to claim my Fitness Membership Incentive?
To claim your fitness membership incentive, you must;
be a full-time regular status employee
be eligible to enroll in medical benefits
be an active member of a local health club or fitness center
provide a copy of a paid monthly membership invoice
submit a complete fitness membership attendance log provide by fitness center/health club reflecting 8 calendar days of workout per month or provide a screenshot or record of completion of a virtual workout that reflects 8 calendar days of workouts per month complete and submit the fitness membership incentive form along with the supporting document

When do I submit my fitness membership incentive form for May, June and July 2020
Quarter 4 (May, June and July) are due on July 31st 2020. Your completed form and supporting documentation may be sent to wellness@swkey.org or faxed at 512.582.9532.
What is the Wellness Program?
The Southwest Key Wellness Program is a work place health program dedicated to providing activities and services that support healthy behaviors and health choices. The Wellness Program offers program incentives that encourage employees to engage in various wellness activities.
How do I sign up for the Wellness Program?
There is no need to sign-up to participate in the Wellness Program; it is a voluntary program available to all benefits-eligible employees.
What is a Wellness Warrior?
Wellness Warrior is our incentive program. Within Wellness Warrior, there are over 20 activities to participate and engage in. By completing, a minimum of just six (6) activities you will earn a Wellness Warrior Sport-Tek shirt, you will have become a Wellness Warrior. Challenge yourself to complete more than six activities. The more activities you complete the more, incentives you earn.

What is the difference between “Wellness Incentives” vs. “Wellness Activity Credits”?
(1) You can earn an activity credit by participating and submitting the required documentation related to any of the Wellness Warrior activities. Wellness credits will accrue towards activity tier levels. Each tier level earns you a various wellness swag/merchandise. See our Tier Levels HERE
(2) Many of the wellness activities earn you a Wellness Warrior credit, but some earn you an incentive for participation. See our Activities Summary HERE
Which Wellness Warrior activities are monetary incentives?
The following Wellness Warrior activities will earn you a monetary incentive and a Wellness credit for participation. All monetary incentives are considered taxable income. To learn more click on the link to download the Activities Guidelines and associated forms.
Is a Biometric Screening the same as a Preventative Exam?
The biometric screening will screen for blood pressure (SBP/DBP), body mass index (BMI), body fat, heart rate, height, waist, blood glucose test, lipid profile (HDL, LDL, total cholesterol, triglycerides) while a preventative exam is a routine exam that identifies potential health problems in the early stage that may be easier and less costly to treat. For the incentive program, they are considered two separate exams.
How do I get a biometric screening?
A biometric screening is part of the healthy lifestyle credit offered by the Wellness Program. To participate, you must be enrolled in one of the Southwest Key’s medical plans. Employees must register with Wellness Corporate Solutions, our biometrics vendor on the WellConnect portal. The portal allows you to:

Download a PCP form for a biometric screening with your primary care physician.

When do biometric screenings begin?
Offsite biometrics start at the beginning of our medical plan year on September 1st. Onsite biometrics start in early November and go through March and screenings are coordinated with each Program Director. The dates and the schedules for which are communicated to employees by email, posters and other marketing collateral. Wellness Regional Coordinators also inform employees of up coming biometric screenings in staff presentations and meetings.
Why are onsite biometric screenings not offered at my program?
Wellness team tries to accommodate every program, but not every program is eligible for onsite biometric screenings. Onsite events must meet the 20-person minimum for onsite biometric screenings.
How I do access my biometrics screening results?
You can contact Wellness Corporate Solutions Customer Support number at 1-877-469-5411.
You can also log-in to your WellConnect portal and download results. Please know that you are NOT required to send your biometrics screening results to Wellness team to receive your incentive and Wellness Warrior credit.
How do I register for onsite biometrics?
WellConnect is your gateway to biometric screening services offered to you on behalf of Southwest Key by Wellness Corporate Solutions.

The following four items are required to register:
Name (first and last)
Date of Birth
Employee ID
Email address
Your Employee ID is the number at the bottom of your pay stub under Miscellaneous.

What is the Healthy Lifestyle Credit (HLC)?
The Healthy Lifestyle Credit also referred to as the HLC is a $25 credit towards your semi-monthly medical premium. Employees can earn the semi-monthly credit by completing a biometric screening. In order to continue to receiving the HLC, employees must complete their biometric screening every plan year. New employees will receive an email communication from Wellness offering directions and information relevant to offsite biometric screenings.
What are the current requirements to receive the Healthy Lifestyle Credit (HLC)?
To participate in the Healthy Lifestyle Credit, you must be eligible for health benefits and enrolled in one of the Southwest Key medical plans (base or premium).
It was my understanding that an online health assessment was a requirement for the Health Lifestyle Credit (HLC).
Starting FY2019-2020 the online health assessment is NO LONGER A REQUIREMENT to receive your Healthy Lifestyle Credit (HLC).
