At Southwest Key, we care about your physical health that is why we offer the Healthy Lifestyle Credit. The Healthy Lifestyle Credit (HLC) was established to encourage participation in a biometric screening with the goal of providing you a health baseline that evaluates risks for a variety of health issues. In addition, if you complete a biometric screening you are eligible to receive a $25 credit toward your semi-monthly medical premium.
Southwest Key does not collect any individual results from your biometric screening and you are not required to meet any benchmark results related to your biometric screening. For any questions, please contact HR Wellness at wellness@swkey.org.

ON-SITE BIOMETRIC SCREENING
On-site biometric screenings are available from November through March at designated programs that have 20 or more eligible employees. Look out for our biometric screening posters, emails and in-person presentations that will inform you on available on-site biometric screenings.

OFF-SITE BIOMETRIC SCREENING
Off-site biometric screenings are available all year-round with the exception of August. There are two options available to complete an off-site biometric screening.

To qualify you must be enrolled in one of Southwest Key medical plans (base or premium)
New employees will receive an email from Wellness offering directions and information on completing a biometric screening once Benefits verifies medical plan enrollment
New employees health benefits are effective the first of the month following 60 days of full-time employment
Medical Benefits are offered at the time of hire and during open enrollment period for eligible employees
Current employees must complete a biometric screening every plan year by July 31st to continue to receive the Healthy Lifestyle Credit (HLC) for the following plan year
Southwest Key does not collect any individual results from your biometric screening and you are not required to meet any benchmark results related to your biometric screening
Individual results will not affect your benefits eligibility
